The Hirsh -Roth stein Group asking for 15. Oo and fill of birth date, and email address, place of birth

I received 2 envelopes -one had a post card in it It was asking me for 15.00 and date of birth and email address.
The same mailing included another envelope from the same address -this one had a letter in it. It was asking for 15.00, date of birth, time of birth, place of birth and email address.

I came to Rip off site and placed the forms of mailing aside until I receive a report what to do next.
