Tried to steal 149.95 from my account

I received notice from my financial institution that Usa Credit attempted to take 149.95 from my account.

No funds were available but I was charged 20.00 fee - because no funds available. I did not complete any application with Usa credit nor give permission for any withdrawal of funds!

Even 20.00 less in an extreme financial hardship. I need this corrected.
