Took my job application and opened a phone account in my name

I applied at the dunedin fl. Gold crown prop mgt for a job in augest of 2008. Today the 21st of feb a collection agency contacted me for delinquent monies to verison phone comp. After investigating it turns out gold crown took my identy and opened up a phone accct. The phone is in gold crowns name and it was put in a apartment and the billing sent to me. That company used my identity.
