Prepaid Visa Card Could Not Be Activated After Loading $40

I Purchased a visa debit card at Walmart, and paid 8.94 for the card. And then I put a 40.00 dollars cash on the card. Here are the numbers of the purchase. St#0866 dp#00004087 TE#64 TR#00237

when I tried to use the card it was bogus, and I also called on a lan line phone, I was told card could not be activated, so now I'm out 48.95.
