I don't know where they got my name from but thank you for this web site or I too may have lost $39.00 for a card that doesn't exist. If one can only order from their catalog that should be included in the offer information. They should also be required to use black ink to print the terms/conditions on the back of their form. Blue ink is the most difficult color to photocopy.
