I found an unexpected charge of $89.50 on my CC. My banking institution account manager gave me a phone number associated with transaction. I called and the receptionist said (today) "You ordered a free sample which comes with an automatic subscription. To cancel you need to complete the forms included with the product... Your order did not ship because we did not receive an address with your order." After some memory jogging, I believe this transaction started when I completed an online survey to receive a "free offer" plus shipping and handling. The BS part is they charged me with them knowing they never sent any product, free or not.
