I am clueless as to how this company got my cc/bank acct.info Internet

I always reject any phone call telemarketers. I am careful with my online purchases.

I wonder how this company got my bank account or cc info.

My biggest worry is that even if this company changes its name, it still has our banking info and can replicate such charges.

Yesterday the website was present. Today it is not. Huh?

My bank suggested that I change my bank account number.
I explained that if I did that I would lose my bank account history as printed on my checks.

I cancelled with "Monique" on the 5th of June after noticing the charge of $14.95 on the 2nd of June. I received a confirmation number.
There was a small charge in May of $1.95.

Adding this up for hundreds of people is quite a total.
