Fraudulent use of Debit card. Unauthorized charges to debit Card

OnlineSupplier.com obtained my debit card information to pay $1.95 for shipment of promotional material and then billed me $49.95 for service that I didn't order. Attempt to contact OnlineSupplier.com by phone and email have failed.
