I am including my follow-up report to my credit card company to show others how this issue might be resolved. Do not mistake any single action as a resolution. It will take time and effort for you to erase these charges.

To XXXX Credit Card Services

While it appears that this issue has been resolved (took 4 hours) I am sending the following in case additional charges are posted by this company which I assume to be FMW LABS as identified on my caller ID.

On 7/24 I ordered the following from Health Products: Nature Cleanse and Acai Berry. Both were trial offers. XXXXX is the credit card transaction number for Naturcleanse in the amount of $. 99. As of today's date, July 29, I do not see a charge for the Acai Berry although the credit card company called and asked if I authorized the nominal (less than $5) shipping fees for samples of the same, which I did authorize. I do not and did not authorize any further charges from this company for any ongoing product orders.

Interestingly, a phone call was received (7/27) from a representative of this company (caller ID: FWM LABS 1-623-238-6401) offering me a free trip to Cancun and ongoing shipments of their products for $39.95 a month. At the end of the conversation I was asked if I understood what the representative was saying. My reply was affirmative. I did understand. However, my next words were going to decline the offer, whereupon the representative hung up on me. I did not authorize further charges to my credit card. I did not authorize ongoing product orders. Another call was received from the same number on 7/29 and I made sure to clearly decline all offers.

In checking on my account with this company via www.myordersupport.com, the system asks for the last four numbers of my credit card, email address, order number and zip code to obtain a password. Upon insertion of that data a notification of the password being emailed to my address came up on the monitor. I have completed that process twice and no password appeared to have been mailed. However, in checking my spam filter I found the email with a password attached. Logging in enables one to check the charges. The live chat enables one to cancel or confirm cancellation. There were charges listed, one of which was an ongoing, monthly $39.95 for a product named Lifecleanse which I did not authorize. After some clarification of my experience and intentions the following information was received from the agent on live chat:
Raimar (CA-003): "Record shows that you have already a cancelled account for the product AcaiBerry Detox which was purchased last 7/23 6:55: 02 PM and was successfully cancelled last 7/29 Here is your confirmation number XXXXX

In calling the automated telephone system at 1-866-949-0138, I have repeatedly (3X) completed the process to cancel my order. The system now recognizes my telephone number and informs me that the order and charges have been expunged. The date is July 29.
