Earlier within the year I noticed 2 physicians that pre-licensed my insurnace overage. 2 weeks before I received EOBis declaring they'd not spend on the basis of the info they'd re: additional protection. 30 days before I obtained a study re additional protection that I used-to condition there's none and shipped back at my cost. This week I obtained a EOB declaring that they're questioning cost for disappointment to supply required info on additional protection, which I'd need to lure for further consideration under ERISA. They've usually but still gather my rates.
