Sent product without by company without my permission and charged my bank acct

I had requested only the free trial product. Now I have received product which I didn't request. I can't find Performance Health USA, LLc on web. No phone number or address. I called a phone number and got a message that number not in service. Please look into this company thoroughly before ordering anything.
I got ripped off!
