Failure to send Coolpix 3700 Rebate of $100 when all requirements were fulfilled

I purchased a Coolpix 3700 because a $100 rebate was offered. I mailed (and have a copy of) 1) Receipt dated 8/23/04 from Digitaltailer 2) UPC code label from side panel of box 3) Serial number label with bar code from side of the box. I was promised a rebate in 4-6 weeks and have not yet received it. I sent a follow-up request 11/14/04 with copies of my original submission.
