Electronic check forgery

Between Jan. 15th and Mar 5th there have been 4 electronic checks written on my account as follows:

$19.95 to the order of SABAWA
$19.95 to the order of SABAWA
$19.95 to the order of SABAWA
$59.85 (3 x 19.95) to the order of C.S., LLC

All checks have the same 'company' phone number in the memo field. The phone number when called has a voice mail that answers:

"You have reached our web sales department. Please leave your name, phone number, email address and reason for your call and one of our representatives will contact you shortly." Then the mail box is full: "Mail box full, mail box full."

All checks were deposited to a Bank of America account in Louisville, KY. The SABAWA checks into one Bank of America account and the C.S., LLC to another.
Account # 11022 12679,08987 81340? - hard to read from the print out of the cancelled check.
