Poor service quality - condiments

I went through the drivethrough at the Lincoln Mall McDonalds location, at about 1:30; The drivethrough wait was long... Not busy... Just a long wait.
I placed my order, and the girl said, "I, sorry, I was'nt paying attention... Could you repeat that?"... So I repeated my order. I got to the first window to pay, and the girl was confused with the money and change... I had to explain to her how to make the appropriate change, based on what I had given her. I proceded to the second window to retrieve my order... Again, they asked me what I ordered. They gave it to me, and I left. I usually check my order, as they often dont get it right. This time I did not. Shame on me, I guess! When I got back to work, and prepared to eat my Southwest Chicken salad, they gave me the grilled chicken, not the crispy I ordered.

They did not give me the packet which contains the utensils and salad dressing. Also, I asked for the iced tea to be "half cut"... It was not.
