Can't reach my Case Manage

I've been given the name of Maria Miquel along with a phone number that is never answered. It rings, clicks and rolls to something else and then disconnects. There is never an opportunity to leave a message.

I need help coordinating some issues, and if this person has been assigned to me for nearly a year, why haven't I heard about it before? This information came from my PCP Dr. Torres.
