Took money directly out of bank checking account without knowledge or consent

When I checked by bank's checking account I noticed two charges of $125.00 had posted to my account.

I never authorized these charges and I am unsure of how they got my bank account information.

I feel violated in that this company was able to get into my personal checking account and take money without my knowledge or consent.
