A $99.49 membership fee was debited to my checking account on 07/02 for Premier Membership Club without my knowing consent. I do not wish to be a member of any on line purchasing system. Please refund in full the above mentioned PMC $99.49 membership fee at the earliest possible time directly to my Zions Bank checking account. The PMC membership fee debit to my checking account without my knowledge or approval caused my checking balance to drop below my available funds resulting in insufficient funds charges of $25.00. Again, I formally and in writing, decline your club membership and expect all funds taken from my account to be electronically deposit back to my Zions Bank military checking account *4488. Wm. T.
