Received the monthly bank statement and a "check" (not mine) was listed for 394.25 with my name and address and the number of my account, and my bank's number. There was no signature, or explanation!
Where this information was snatched from I have no idea. I called a tel number that was on the "draft" and was informed that was an over the phone call for medical ins and I had given consent. No way would I do that, ever!
