I fell on a nonvisible water spill in Isle #4, the frozen food isle. Ayellow cone was 18" away from the spill. I hit my head in the back. Hit both shoulders, and fell on my back and twisted both knees. The assistant store manager Sandra Tucker and head cashierclerk Marie came to help me. I could not get up, and a man customer helped me up. The assistant store manager took myname and did not fill out a report there and now. I was contactedafter numerous calls by me, by Dennis Rose. He sent me a reportto fill out (arrived Oct 3). I have had 3 doctor visits and6 physical therapy treatments due to a Frozen Left Shoulder. My insurance is paying all my medical bills. My claim was deniedbecause Safeway put a yellow cone in place. However, the waterspill was still there, and the head clerk Marie told the clerk to putpaper towels on the water spill and clean it up. This was not donebefore I fell. I feel that this was Safeway Stores fault. I did not ask to fall, and have not fallen before in any stores. I would like $2000.00 for pain & suffering (which is the amount of my medical expenses)
