I purchased a cell phone from Costco on 11-11-08. If I filed the appropriate online or hard copy forms (which I did), I was supposed to be reimbursed for the $35 activation fee I was charged to activate my new cell phone. Also, if I allowed T-Mobile to bill my American Express card each month and as long as I filed the appropriate online or hard copy forms (which I did), I would receive a $10 cash card from Costco. It has been over four months and all three companies are blaming each other. I truly believe it's Costco's responsibility and they owe me $45. I've spent 4-5 hours on the phone speaking to people at all 3 companies with only promises of callbacks I've never received. I am barred from saying what I really would like to so I'll leave it at that.
