Check Conversion Voucher from a company called Alliance. I didn't notice it as it was listed with my checking statement along w/the photo copies of checks I had written. As I only noticed it first of February my bank says I'm burnt! In process of closing my account. Don't know why the bank allowed as it doesn't even have the correct routing number.
