Charged me and asked my phone company to add it to my monthly bill City

Got my Qwest bill and it went up by almost 15 dollars. There was a new monthly bill by someone named ESBI, Orbit Telecom. I never ordered any service, and never gave anyone from Qwest permission to ever add anything to my bill.
