I was sent an "Offical Prize Claim Form" from Michael J. Armstead, Prize Claim Coordinator, that I had won $4,687.63 and to claim it. However, what I realized upon trying to do so is that they failed to send me the results so that I could put the appropriate TSN no.in the claim form in order to collect it. They never sent them to me. I know because I check everyday to see results. So, they wrote me back and told me they were not going to pay me my winnings. I kept the official claim form and will be happy to send it for verification of this fact.
