I had received this Claim Form stating that i had credit insurance from life Investor insurance Co. For the total amount of 608.48 by submitting a early payoff document ion like a letter, contract stamped paid in full, such as title transfer, lien release. Bottom line if you cannot obtain such documentation you can sign the Claim form verifying that you paid off your loan early and furnishing the approximate date of the early payoff or refinance, which, if approved by Rust Consulting, will entitle you to receive a $100, refund of your unearned premium. Then is said that claim may be audited. If you fail to submit your fully completed and signed claim form with or without an accompanying document showing early termination, and postmarked no later than December 15 or your claim will not be timely and you will lose your right to receive a refund.
