National Union Fire Insurance This Company Got electronic withdraws from my U S Bank checking account without my Permission

U S Bank of Fond du Lac WI gave this insurance Co. My checking account routing numbers to electronicly get $11.00 a month taken every 3 months at $ 33.00. I guess there was a year free on U S bank and with proper notice you would start paying for this accidental death and dismemberment Insurance of $200,000. I have no policy in hand or anything else to show i really have this so called insurance? The Bank Will not admit that they are at falt in this case. So needless to say i am dumping U S bank this week! I only lost $99.00 but i am very angery at the bank for allowing this to happen!!!
