Innaccurate Past Due Account Falsely Reported on my Credit Report

I do an annual credit check every year and recently I found a charge for $884 from AFNI for Dish Network. I do have an account with Dish Network, but it is in great standing and I've had this account for over a year. I've submitted multiple requests to get this removed and it is still on my credit report! I dont know what to do.
