Waiver form for uninsured motorist coverage rejected twice despite full payment

When I signed up for Motorcycle insurance with Geico, I signed up for the least expensive policy they had with the minimum insurance requirements... $15K bodily Injury and 30K property damage, and I told them that I do not want the uninsured motorist option because it cost more money. They acknowledged my request, accepted my full payment for 12 months insurance, they just wanted me to sign a waiver form for the Uninsured Motorist. I signed the waiver form and sent it to them and then two weeks later I rec'd a phone call stating that I had not filled out the waiver form correctly, and that I would have to do it a second time... So they sent me a second form, and I called Geico to assist me in filling out the form, but apparently, even with their assistance, it was filled out wrong. I had clearly checked the box stating I do not want Uninsured Motorist added to my policy, and Geico even acknowledges this on my insurance policy paperwork.
