Unauthorized charge appeared on my visa debit statement, unable to resolve by phone, had to refute charge with bank

An unauthorized $34.14 charge appeared on my bank statement as:

VISA PURCHASE GRC*WEN BY CHAZ DEAN 800-7281103 CA US.
I had made no purchase nor received any product.

I called the 800 number but was only able to leave a message.

Four times.

The party would not explain how they got my account number nor were they refunding.

They indicated the ship to address was Canada, not my address, and unlike most reputable dealers, shipped to an address other than the cardholders.

I filed a dispute with my bank, cancelled the Visa card, and requested a new one.
