I have been scammed by aflacassist.com, llc. Address on a package I received 4 days ago is Aflac Assist.com LLC, 3507 N Central #503B, Phoenix, AZ 85012.
Names I have are William Ferguson and Tom Bradford both at 1-800-354-0274.
They also use US Document Assist LLC (same address) to copy and have me sign various docs-email from Anna Hobbs.
I invested $219 for an EIN which I received from IRS unless the letter head was bogus. I then gave them $3000 for advertising which was supposed to pay me thousands of dollars. This is all since 8/9.
Customer Ervice is 1-800-875-0661 (Suzanna). Merchant Solutions is 1-602-753-9758 (Wendy Diaz, she also signs Customer Service). My "Coach" is Tom Gilbreath at 1-602-413-9469 (cell phone he says), email: financeandcash [email protected].
Another number I see on letterhead is 1-800-918-2640 (same address - 3507 N Central. Cust, Service email is [email protected]. Another address I see on letterhead is Aflac Assist.com, llc, 13416 N 32nd Ste 102, Phoenix, AZ 85032, ph 888-882-2071. Two other phone's from a copy are 1-866-864-2275 and 1-602-464-5099 with the N Central address.
They gave me 2 websites, multiple letters & various DVD's.
