Sir/Madam, This bill has been falsely charged and we have disputed time and again with no solutions.
At the time of the visit, ck #1372 for $50.00 was paid on Jan, 18th 2010. A copy of this cancelled ck was sent to you before.
As per the insurance company, this visit was charged as an emergency visit which they too denied and paid Baylor the appropriate amount.
After a numerous calls, your unorganized billing office has a different views each time.
My only alternative to solve this is to involve a third party; ATTORNEY GENERAL
