Is still being run out of a P.O. Box in Washington. They are still asking for a $45 deposit to become a home mailer but do nothing more than take your money. Looking at the details within the PDF file sent as an application I found the name Nichole Mura, who is probably purporting the scam.
I am in the process of contacting law enforcement agencies in washington, new york and with the government to inform them of this scam, but until then be careful, and do not send any money to the address they give you. I am also contacting comcast, who is providing webspace for the application and website that you are directed to via e-mail.
