I took my daughter in and they wanted me to fill out a form that required me to write out my credit card or debit card information and sign it to allow them to deduct what the insurance doesn't pay. I have no problem with that but I am nt writing it out for any employee to have accesss to. So I told her I would pay cash for the visit and my insurance could just pay me. She told me no they don't don't do that.in order to see the doctor you had to fill out that form. I said I will pay cash. She said they would nt see her. I then asked for the other form I filled out with SS# on it and she said no. I said you are not seeing her so you don't need it. She went to the scanner an scanned it in. After a long argument she gave it to me. But it's scanned in the computer so it wasn't any big deal to give it back. I think this is shady to say the least don't give them the opportunity to scam you.
