I am enclosing documentation for the investigation conducted for the co payments on my Walgreens medications for a whole year. It is missing the documents for January, February and March for you to submit and review.
Some time ago I submitted a verbal complaint to your Mrs. Patricia and she tried to help. She said a check will arrive but it never happened. At this point the amount of said check is bigger because there are a lot of copayments I have paid that needs to be revised and return the money back to me.
Please attend this case as soon as possible, it is very important that everything is cleared between the parts for the confidence and trust in doing business with me.
