I cancel twice but they keep charging my credit card. Phone no longer in use Las Vegas

I have canceled this account with them twice once on nov. 5-06 and the second time on Dec. 01-06 but they keep charging my credit card each month.
I have tried to reach them by phone today 12-23-06 but their phone is no longer in use.
