I think Premier Health Plus is somehow related to Chase. Chase should take the responsibility too. If not, how did it get my credit cards. When I called Chase, the lady quickly told me that she could transfer me to the company and canceled my membership. Why couldn't Chase cancel it for me? It got my credit card number from Chase. BAAADDDD business practice.
