They billed me multiple times unauthorized with a name similar to my credit card name

Beginning in January, without authorization, AtHomeRewards began billing my REWARDS credit card $19.95 a month. I didn't notice it because the name of my credit card was "REWARDS" and I assumed it was part of my credit card service, which has an "awards" program. As I am 85 years old and a bit too trustworthy, I was not suspicious until I began calling the phone number on my statements and was constantly put on hold. I then reported it to my credit card issuer, and they are sending me an affidavit to fill out.
