Mr. Hero in Parma. OH on Brookpark road jipped me the only two times I have been there. The first time they never gave me the $0.60 cheese and this recent time (Server name: Mike, Order #22, sunday, December 04, 8:26 PM, Receipt #1510321) I was charged $2.49 for Cheese Bites which I never received. Horrible experience if trying to get your money's worth.
