I was charged a fee of $4.95 and never heard of this company Indianapolis Indiana

I was charged a fee of $4.95 on June 19th. I have never heard of this company, nor had I authorized this and the phone number provided is no longer in service (330) 871-7932. I would like this amount refunded immediatley and however they received my information, I want it taken off their list.
