12/4/07 Purchased D-9 Krzak Road
Front bedroom has 2 missing vertical blind pieces; Dealer said she ordered from Skyline
Furnace fan motor extremely loud during operation
12/5/07 No Hot Water in bathroom showers; water was cold in one bathroom and lukewarm in the other bathroom.
12/6/07 Regarding telephone line there was no pull string in it, so they could not put the telephone lines through the conduit; they installed the telephone cable above ground (laid line on the ground) and connected it to the junction box on the east side of the building; Cox cable had a problem because there was no pull string in thereother techs came in and used a fish to pull the wire through and they could not find the terminal where the cable is connected to the home; I called dealer and left a message about this he gave me the phone number to call at Skyline I called & the person told me where the terminal cable connection should beit was hidden in a plate near the water heater at the rear of the building. It should have been identified better.
12/12/07 Contacted dealer (Judy Krzak of Krzak Mobile Home Park) re hot water issue; she asked if we had read the MANUAL; when we kept complaining, dealer contacted a plumber; the plumber visited our home and made the necessary adjustments
12/26 Called and spoke to Tom Thacker @ Skyline in Fair Haven, Vermont
1.) In regards to the excessive noise from the gas burner.
2.) In regards to the smoke alarm intermittently beeping twice (two beeps) in the front room.
3.) In regards to the rear door not closing properly. Screen door hard to close. It binds.
He said he would set up a service call.

1/2 Furnace service call placed by Skyline
1/7 Technician visited to look at furnace; he replaced the blower motor and defective smoke detector; he adjusted the back door; he brought a missing blind for the front bedroom (still have one missing blind piece)

01/14: Called and spoke to Tom Thacker аbout:
1.) Dust in home.
2.) Crack in shower stall.

2/7/08 Fiber Pro visited to correct the crack in the shower
2/12/08 Skyline sent technician to clean duct system; he pulled the registers and vacuumed out the ductshe vacuumed out a half gallon of dust; tech also looked at siding which was discolored from material leaking out of gutter; wrong size blind was brought for front bedroom window
2/18/08 Called Tom Thacker; did not return call; placed second call re air quality
03/05: Called and spoke to Tom Thacker in regard to continuing issues to be addressed:

1. Dust in home (air quality is very bad; we must run two air filters 24 hours a day; have much congestion with difficulty breathing)
2. Replacement blinds for the front bedroom window.
3. Rear door has cold air coming in.

4. Waiting for a new window blind for the bedroom.

He will call me back for an appointment. He said air quality/dust is not covered in warranty.

3/15/08 Letter sent to Ronald Foster of Skyline, Indiana with chronology of contacts with Skyline in Vermont

3/24/08 Letter sent to Ronald Foster, Skyline, Indiana re air quality problems in our home and health problems or Herchell and Pat; we requested that Skyline find the root cause of the air quality problems

3/25/08 Ronald Foster sent email that he had been in contact with Greg Cummings, Service Manager of Fair Haven Division; he would be formulating a plan of action; a crew would be sent to our home to check the underbelly of our home to gain access to the air ducts; he said the underbelly would be resealed after they checked out the underbelly.

3/25/08 Herchell sent email to Skyline indicating that he did not want the integrity of the seal broken under the home.

3/25/08? Skyline employees came to the house to assess the situation; They took pictures of the dust and building residue from the air ducts. Herch would not allow Skyline to break the seal on the underbelly. The air ducts had already been cleaned out; there was no need to go under the house and check the air ducts. The problem is coming from somewhere else.

3/26/08 Advanced Air Quality Inc. (Ed) came to clean the air ducts, but because we wanted the bag of debris which would be gathered, Skyline would not agree to this. They wanted the bag of debris. Due to this dispute, Advanced Air Quality would not agree to clean the ductsthey did not want to be involved in that dispute.

3/26/08 Herch sent email to Skyline requesting authorization to have all fabric / clothing cleaned, and the entire inside of the house cleaned to remove sheetrock dust from the inside of the house. Also, the air ducts need to be cleaned again. We need to find a company to clean the air ducts. Herch contacted Buckley Heating and Cooling to have the furnace cleaned to remove the sheetrock dust. Herch sent an estimate from Lynchs Cleaning and Restoration to clean the inside of our home. He requested authorization to have the furnace and inside of the home cleaned.

3/27/08 Herch sent an estimate from Home Depot for air filters and replacement filters needed to run air filters in the house.

3/27/08 Herch sent Skyline estimate from Lynch Cleaning for cleaning inside of house and carpets.

3/27/08 email from Skyline/G Cummings re Skyline attempting to resolve our concerns. Because we refused to allow inspection of ductwork under our home,
Skyline was not able to identify scope of our concern. They agreed to professional air duct cleaning, professional home cleaning and profession clothes cleaning. They requested estimates for the work.

3/27/08 email to Ronald Foster of Skyline; per telephone call with Greg, Lynchs cleaning bill is approved. Skyline indicated they would not release any funds until we gave them all the bills paid and signed a release. Skyline refused to pay the bills as we submitted them. Skyline refused and said we needed to send them all the bills after we paid them and they would send us two payments and only after we signed a release.

3/27/08 Buckley Heating & Cooling cleaned furnacebill $157.50

3/28/08 Letter from Treadway indicating they agree to reimburse us for prepaid invoices for cleaning services, provided estimates are for approved Skyline services being rendered. They will reimburse in 10 days of receipt.

3/29/08 email to H. Treadway re problems with back doorstorm door needs repair; front storm door needs direction of opening changed. It opens on north side and the wind is blowing it off its hinges.

3/31/08 email from H Treadway re storm door at back of house. Looking for information re problem areas. We gave him the information. He said he would act on this as soon as possible.

4/15/08 Skyline send email that service will be calling next week to set up a date to change front storm door.

4/18/08 email to H Treadway/Skyline re dust accumulating in front bathroom again; need to have bathroom cleaned again; upon cleaning the exhaust fan and then turning on the fan, the fan was blowing dust down into the roof from the vent in the roof, rather than drawing the dust out of the house. This needs to be addressed.

4/25/08 email to Skyline re reimbursement of Lynchs Cleaning and CRDN cleaning; email from H Treadway; bills have been approved for payment.

4/30/08 Herch appointment with Tru-Care for physical therapy; tight neck and back due to stress co-pay $15

5/1/08 email from Herch to Skyline re service call. Dust is till flying around in the bathroom and bedroom. Herch asked when Skyline informed Krzaks of severity of dust problem here. Skyline informed us they sent Judy a note on 1/28/08 about dust problem and a missing blind. Harry said he and the service department were moving forward to correct any issues we had.

5/2/08 Herch had appointment with Tru-Care for physical therapy.Co-pay $15

5/5/08 email to Harrytroubles continuing. Dust accumulating in bathroom and bedroom. HEPA filter replaced in living roomcompletely full of fine powder. Replacement was in less than a month. Bedroom smoke detector went off todayfeel it because of the fine dust. Still experiencing health issues. Will contact someone who can identify where the dust is coming from. If problem continues, we will contact the building inspector or health department and ask that the building be condemned.

5/5/08 ltr from Treadway re dust accumulation. He said if there are problems with the services provided by the cleaning companies, we will have to address those problems with them. They believe they acted in good faith while responding to our demands. Any dissatisfaction now needs to be directed to the service providers we selected.

5/13/08 email from H Treadway re Herch not allowing service people access to heating ducts at underbelly. He said he did not agree to pay a bill for Dakota Labs. Service said the work done inside our home was complete and we were satisfied with it.

5/14/08 email to Treadway re front storm door; door is defective; when raising storm glass, metal clip is missing on one side and door falls down; screen is buckled and coming apart; molding strips in shower in front bathroom are loose and were not caulked or glued down; also molding behind toilet was not glued or caulked

5/14/08 email from Skyline; will try to schedule service in next 2 weeks

5/14/08 electrical problem; in turning on living room ceiling fan switch, as my finger left the switch it inadvertently touched the face plate below the switch and Herch received a jolt of electricity not a static shock. At least 5 amps. Similar to a jolt due to a poor ground.

5/15/08 Airhart Electric checked out electrical system. They said the proper wiring system was never installed. No ground wire. Will need to rewire the incoming service. Herct sent Harry an email re above. Bill $166.00

5/19/08 letter from Airhart Electric to Kingstown Mobile Home Park & Sales; George & Judy Krzak re electrical violations at D9 Krzak Road. There is no ground conductor between the main disconnect at the meter pedestal and the electrical panel in the mobile home. This violation is causing the owner of the mobile home to get static voltage shocks frequently. This is a safety issue and a violation of the National Electrical Code. A grounding conductor needs to be run between the main disconnect switch and the electrical panel. Krzaks response was that others had wired the service and that the service is wired properly and grounded properly. Krzaks stated they would have their electrician confirm that all was properly wired and grounded. He advised that his findings on 5/16/08 is that the service is not grounded properly and this is a serious situation and this violation needs to be corrected immediately.

5/20/08 email from Cummingsre storm door is this the door that Morrie installed. We responded yes. Looks like an old used door.

5/21/08 email from Treadway re electrical; he will have bill paid and he is talking to park owner about hook up. He said that a certificate of occupancy was issued which means he had an inspection done and the installation was approved.

5/27/08 email to H Treadway from Herch; dust is still heavy; problems with air quality even though we have 3 air filters working 24 hours a day; storm door is useless. We will hire someone to install a new storm door and send Skyline the bill. Waiting for lab results from air quality test. Will send quote from company which will try to locate and remove and reinstall faulty installations of sheetrock and also strip seal and repaint the walls. We suggest they give us a new livable home. This unhealthy situation cannot go on.

5/27/08 email from H Treadway. Storm door was supposed to be a new one. Do not hire someone to install or buy a storm door as I cannot approve anything without looking into what has transpired.

5/27/08 email from Herch to H. Treadway. Continuing to have health issues. Need a new home. Skylines reputation is at risk. Problems have not been resolved.

5/29/08 Pat email to H Treadway; advise as to reimbursement for 2 bills: Dakota Labs 1750 5/12/08; Airhart Electric $166.00 5/15/08; also broken storm door not repaired. If no response is received, we will hire someone to either fix or replace the door and screen. We are waiting for Dakota Labs further report regarding air sampling and analysis as to the dust problem which hasnt been resolved in our home. We will email the report when we receive it. We are continuing to experience air quality problems and running 3 filters 24 hours a day. Our air conditioner cannot be hooked up per our electrician due to the dust problem. We need to resolve these problems ASAP.

5/30/08 received report from Dakota Labs re air quality to determine state of the indoor environment with respect to dust-like substance, to determine the source of the substance, how much is in the environment and a plan of action to remove the irritant.
Conclusions: sheetrock dust is in all locations tested. Highest concentration of the dust is in portion of home containing the furnace (office and office bathroom area). The HVAC system is drawing the dust toward the environmental control system. The levels of dust are having an adverse effect on the occupants of the home. It is reasonable t o require a reduction of the offending irritant just as the National Heart, Lung and Blood Institute does with the reduction of asthma allergen exposure to lessen the number and severity of asthma attacks. Past cleaning is not the type needed to isolate end remove a particular allergen. Cleaning should be addressed as to sheetrock dust irritants. A cleanup recommendation was given.
