The mailing address on the letter I received, dated 3-3-06 or 3-4-06, is identical to the LVAAP letters reported elsewhere, and uses that name on the form. However, the name on the envelope is U.S.A.P.A. - the PO box is the same, 98855 - but the ZIP code is different - 89193-8855. The forms are ...
