Medifast, OPTAVIA and Trilivy: what changed, how to cancel or return, and where to complain (checked October 18, 2026)
People searching for Medifast today usually mean the weight-loss programme sold by Medifast, Inc. under the OPTAVIA name. On July 17, 2026 Medifast filed a Form 8-K announcing 'the rebranding of OPTAVIA, the lifestyle brand and a wholly-owned subsidiary of the company, to Trilivy'. The company's own pages, opened on October 18, 2026, now use the Trilivy name, and optavia.com redirected to trilivyhealth.com when checked. Your order emails, card statements and account may therefore show either name. This page reports the company's published routes; your own order terms control.
Contacting support. The Trilivy contact page says 'Text us at 1-206-828-1605 for support' and links a web form at clientsupport.trilivyhealth.com; the Client Support page repeats 'Text: 1.206.828.1605'. No voice number or e-mail address was listed on those two pages (checked October 18, 2026). Separately, the Premier+ terms say 'You may contact the Client Support Team at 1-888- TRILIVY' (the numeric form was not shown), and the Terms give a dispute e-mail address, [email protected]. The Trilivy contact-us address itself returned an error page when checked.
Autoship (Premier+). The Trilivy Premier+ terms describe 'an autoship program' and say clients 'will be automatically charged for and receive an TRILIVY Premier+ order each month, unless You modify Your order, change Your ship date or cancel'. The same terms also say 'Every 4-week increment when Your TRILIVY Premier+ order is processed, Your credit card on file will be charged', and the Client Support Premier+ article says 'autoship will run every 4 weeks (28 days)'; both the monthly and the 4-week wording appear on company pages. Enrollment is described as selecting 'the designated check box in the checkout stage' or 'by texting 1-206-828-1605'. The terms also state 'Effective October 1, 2026, a standard shipping fee of $12.95 applies to all orders shipped within the contiguous United States' ($14.95 for other destinations). They state: 'All modifications and cancelations ... must be submitted electronically via Your online account or via phone no later than 6:00 p.m. ET the day before Your TRILIVY Premier+ order processing date', and later requests apply to the next scheduled order. They also say 'You may cancel at any time' and 'Refusing or returning an TRILIVY Premier+ or On Demand order does not automatically cancel Your TRILIVY Premier+ enrollment'; you are 'financially responsible for all orders shipped ... up to the date Your cancelation takes effect'. The terms say there is no membership fee. The terms link a 'TRILIVY Premier+ Cancelation Article'; that link points to the legacy host answers.optavia.com, which did not load on October 18, 2026 (certificate error in a browser and in a script check). The routes stated in the terms are the online account, phone, or the text line above.
Returns and refunds. The Trilivy return policy says 'you may return the consumable products in your order for any reason; non-consumable items are non-refundable'. Consumables must be returned within 30 days of receipt (postmarked within that window); shipping fees are deducted; refunds are generally processed within 2-4 business days of receipt and go to the original payment method; once processed, clients 'generally see their refund within 1-5 business days'; exchanges are not permitted; the prepaid return label or QR code option 'will be deducted automatically from their refund'; for discounted Premier+ orders, clients 'will only be refunded the amount actually expended by the Client for the discounted order'; free promotional items and the Trilivy Certification are not refundable; clients pay return shipping. The page shows no 'last updated' date. Medifast's 2025 annual report separately states that its policy allows returns of consumable products 'from the time of order until 30 days'.
Disputes. The Terms and Conditions are governed by Maryland law and contain arbitration and class-action/jury-trial waiver sections; they tell users first to e-mail the company about a dispute and attempt informal resolution for sixty days. The Terms name arbitration 'BY THE AMERICAN ARBITRATION ASSOCIATION' under its consumer rules and say the client is responsible for 'THE FIRST $200.00 OF SUCH FEES'. They also say that if 25 or more clients send similar dispute notices, the claims proceed in a coordinated bellwether arbitration. The Terms state that you may opt out of the arbitration agreement 'within 30 days after using the Sites or making a purchase through the Sites' by letter to Trilivy Legal Department, 1501 S. Clinton Street, Suite 500, Baltimore, MD 21224, 'stating Your name, Trilivy Client account number, and your intent to opt out'. Read the full clause on the Terms page before relying on this summary.
Outside complaints. You can report a company to the Federal Trade Commission at ReportFraud.ftc.gov, which invites reports of 'bad business practices', and dispute a card charge with your card issuer. The BBB profile for Medifast, Inc. displayed 0 complaints and 0 reviews on October 18, 2026.
A different Medifast. The domain medifast.com is not this company: its home page, opened October 18, 2026, describes 'MediFast', 'a professional Pre-Insurance examination provider' founded in 1991. Its footer reads 'Copyright © 2026 MediFast Inc.', a different company from Medifast, Inc. of Baltimore; do not confuse the two.
Who operates Medifast: Medifast, Inc., Baltimore
Medifast, Inc. is a Delaware corporation listed on the New York Stock Exchange (MED), with its principal office at 100 International Drive, 18th Floor, Baltimore, Maryland 21202 per its fiscal 2025 Form 10-K. The 10-K describes the company as 'the 40+ year old health and wellness company' and says its network had approximately 16,100 actively earning independent coaches for the quarter ended December 31, 2025, and that approximately 90 percent of coaches were clients first (company statements). SEC records list its former name as HealthRite Inc. until 2001.
The OPTAVIA brand was a wholly-owned subsidiary that sold the company's Fuelings (meal replacements) and coaching programme through independent coaches. The July 17, 2026 press release says 'the transition from OPTAVIA to a new brand with the launch of Trilivy' is an evolution 'beyond traditional weight loss' to metabolic health, and introduced new meal replacements. Nick Johnson is named as CEO in that release. The Terms of the Trilivy site name Trilivy, LLC as the contracting company; Exhibit 21.1 to the 10-K lists OPTAVIA LLC (Delaware) as a subsidiary, and the sources checked do not state how Trilivy LLC relates to OPTAVIA LLC, the defendant named in the lawsuits below.
Reported results (company filings). Fiscal 2025 revenue was $385.8 million, down 36.0 percent from $602.5 million in 2024, with a net loss of $18.7 million (10-K). For the three months ended June 30, 2026, revenue was $76.4 million versus $105.6 million a year earlier, the net loss was $3.1 million, and actively earning coaches were 11,700 versus 22,800 (10-Q). These are the company's reported figures, not our assessment.
For people considering becoming a coach, the become-a-coach page says Trilivy 'makes no guarantee of financial success' and refers to a Trilivy Income Disclosure Statement; a link to it was not found on that page, and the obvious addresses returned 404 on October 18, 2026.
Older consumer-facing channels included 'Medifast Direct' (online) and franchised Medifast Weight Control Centers, which the fiscal 2018 10-K lists among the channels that were folded into a single OPTAVIA reporting segment in March 2018.
FTC consent decree with Medifast subsidiary Jason Pharmaceuticals (2012)
On the docket of United States v. Jason Pharmaceuticals, Inc., a Maryland corporation 'and a subsidiary of Medifast, Inc.' (D.D.C., Civil Action No. 12-1476), a Consent Decree is filed as Document 3 on September 17, 2012. The decree says it was entered 'without adjudication of any issue of fact or law and without Defendant admitting liability', and that 'Defendant does not admit or deny any of the allegations set forth in the Complaint, other than jurisdictional facts'. The parties agree that it 'settles allegations in the Complaint of violations of a prior consent order, entered in FTC Docket No. 3392'.
Terms read on the decree: a civil penalty of $3,700,000 under Section 5(l) of the FTC Act; and permanent injunctions, for any 'Covered Program' (low-calorie meal replacement programmes including 'The Medifast 5 & 1 Plan'), against representing that users can generally expect an endorser's results, or that a programme enables users to lose any particular amount of weight or maintain loss, unless the claim is non-misleading and supported by competent and reliable scientific evidence as the decree defines it (including at least one adequate and well-controlled human clinical study, or a study meeting its Appendix A criteria); and against unsubstantiated health, safety or side-effect claims. The decree applies to the Corporate Defendant and 'its successors and assigns'. Medifast's 2025 10-K also refers to this 2012 decree. The sources checked did not show any later FTC action against Medifast.
Complaint record: BBB, Trustpilot and archived complaints on this site
BBB (as displayed on October 18, 2026). The BBB profile for Medifast, Inc., 100 International Dr Fl 18, Baltimore, shows 'NOT BBB Accredited', a rating of A, 0 complaints and 0 reviews. BBB displays a rating reason of 'Failure to be transparent about ownership, location, or products/services offered'. From the profile's raw data: incorporated 1981-07-23, business start 1989-01-01, file opened 2004-06-07. BBB also lists a CEO (Daniel Chard) other than the one named in the July 2026 press release (Nick Johnson) and legacy websites (tsfl.com, ChooseMedifast.com, medifastcenters.com), so some BBB-listed details may be out of date; the (800) 209-0878 number is BBB-listed only.
Trustpilot (as displayed on October 18, 2026). The page for optavia.com shows 34 reviews and a score of 1.8, 'Unclaimed profile', and 15 reviews in the last 12 months; Trustpilot notes that the company has no history of asking for reviews, so reviews may not be representative. No Trustpilot page for trilivyhealth.com was found on October 18, 2026 (the address returned a page-not-found message). Review counts are not combined with the figures above or with our own archive.
This site's archive. Two approved complaints from 2009 and 2010 name Medifast as the subject: a June 2009 post (listing 'Take Shape For Life - Medifast Diet Food') says the meals taste bad and that exchanges are allowed only within 30 days with no money-back guarantee; a July 2010 post says an exchange of ten items was delayed and customer service did not escalate. A third post (February 2012) from a former health coach of 'Take Shape for Life', linked as a non-primary mention, says about $14 owed to the writer was unpaid. These are unverified user accounts from 2009-2012 under programmes and policies that differ from today's; they are not findings.
Court cases involving Medifast and OPTAVIA
Medifast's 10-K and 10-Q say the company is 'from time to time, subject to a variety of litigation and similar proceedings' arising in the ordinary course and does not expect a material adverse effect; neither filing names a specific pending consumer class action, and the June 30, 2026 10-Q did not contain the words FTC, class action, subpoena or investigation.
Cases found in the sources checked: Douglass v. Optavia LLC, an ADA class action over website accessibility for blind screen-reader users, settled with injunctive terms (final approval orders reported January 23, 2023); Zeller et al. v. Optavia, LLC, a California Automatic Renewal Law class action over Optavia Premier enrolment, listed by Truth in Advertising as settled and dismissed without prejudice; Proter et al. v. Medifast, Inc., a 2011 securities class action dismissed in 2013; and the 2012 FTC decree above. Details are in the case list. No dollar amounts for the two OPTAVIA settlements were found in the sources opened.

| Douglass v. Optavia LLC Docket 2:22-cv-00594-CCW | U.S. District Court, Western District of Pennsylvania | Settled | Settled Class action alleging the OPTAVIA website was not accessible to blind and visually disabled screen-reader users under the ADA. The settlement notice site says Optavia LLC 'agrees to take additional steps' to make its website and any new website or mobile app accessible, with no monetary amount shown; it lists an objection deadline of December 20, 2022 and a final approval hearing on January 23, 2023. Mealey's (January 24, 2023) reports two orders dated January 23, 2023 granting final approval. The notice gives the case as 'Douglass v. Optavia LLC, No. 2:22-cv-00594-CCW (W.D. Pa.)' and the complaint on the settlement site is stamped filed 04/21/22. It says class members 'are not releasing any claims for monetary damages', the named plaintiff may receive a '$1,000.00 incentive award', and class counsel may seek fees up to $45,000.00 plus two further possible $15,000.00 amounts. The final approval date rests on the Mealey's report; the docket itself was not opened. The Trilivy site still shows an 'ADA Class Action Lawsuit Settlement Notice' banner. | Source | |
| Zeller et al. v. Optavia, LLC Docket 37-2024-00013298 | California Superior Court, San Diego County | — | Dismissed | Dismissed Putative class action under California's Automatic Renewal Law concerning Optavia Premier enrolment; a Venable article (March 26, 2024) says it was re-filed in state court after an earlier federal filing and describes the allegations (coaches described a one-time order, a pre-checked Premier box, undisclosed renewal terms). Truth in Advertising lists its status as 'Settled and dismissed without prejudice' and a filing date of March 2024; the San Diego docket was not opened, so no court record of a settlement was seen, and settlement terms were not found. These are allegations, not findings. | Source |

Docket 2011-CV-720[BEL] | U.S. District Court, District of Maryland | Dismissed | Dismissed Securities class action alleging violations of Sections 10(b) and 20(a) of the Securities Exchange Act and Rule 10b-5; consolidated with a related case on July 19, 2011. Per Medifast's 10-Q for June 30, 2013, the court granted the company's motion to dismiss as to all claims on March 28, 2013; the filing states no later appeal. Not a consumer case. | Source |
