UPS I asked for and thought I purchased insurance on my package but it doesnt state it on the form

On October 10, I stepped into my local UPS store, like I have done numerous times. I stepped up to counter with my box that was already packaged correctly by DELL INC, and proceeded to talk to MATT about the right service that I needed to ship to an APO.

We discussed the different types of shipping methods and different types of insurance coverages. I felt very confident with MATT. So he asked if I would like extra stuffing the box with the camera, I said yes that way I could also put a few pieces of mail in with in... Trying to kill two birds with one stone. So I thought.

I was MATT puppet I signed the forms and signed everywhere I was told to sign, just like always. The form that I signed was the Custom Declaration and Dispatch Note by the United States Postal Service... No problem right?!

I had to remind MATT that I wanted Insurance on the package... The camera is worth $4,500 USD... The first price that he gave me was not with the insurance and that price was for $32.25 which is on the form, he explained what the coverage was and everything. So he added the insurance, so I thought... The price jumped from $32.25 to $53.75 which is the price on my UPS receipt.

The package has never been delivered to the addressee and after looking at the form I realized that the section where the insurance coverage goes its BLANK... MATT is to blame for this... I have called UPS and Unites States Postal Service asking for and investigation, and have came up short...

Its sad because this camera is hobby of US Military man, this hobby gives him sanity in such a hectic environment... He is stressed and Im stressed, I don't know what to do. Of Course they are claiming that it is not insured and Customs is claiming to never have the package...

They have failed me in such way... They will not explain to me why there is a price change on my receipts... I know this package was insured I paid for the insurance and now I get nothing... Not to mention the Parcel Indemnity Coverage, I have called several times and nothing has happen. What oh what shall I do...
