Dear Sir,

Sub: Repair of System 2450 Diathermy / Cautery Unit, Sl No: 08FGU064Ref: Tax Invoice No: 57 Dated 24/11

We have purchased 2 numbers of Conmed Diathermy/Cautery Machines form you bearing Sl No: 08FGU064 and 08JGU062 valued at Rs. 6,80, 000/- (Rupees Six Lakh Eighty Thousand Only). Accordingly this amount was paid to you on 28/08 and 05/01. You installed the machines on 24/11. After the installation, there were frequent problems with the machine bearing Sl No: 08FGU064 and we were contacting you on phone to rectify the complaint of the machine. However you did not come and inspect the machine inspite of several oral reminders. It was only on 03/08 you came to our hospital and check the machine. Your service engineer Mr. Ganesh informed us that he has to take the equipment for repair. He was allowed to take the machine for repair. He returned back the equipment and promised that machine was serviced. When put to use again the machine did not work. Again it was on 27/01 your service engineer Mr. Susanth came and inspected the machine and again took it for repair on 25/01. He never brought back the machine, which he took it for repair.

Meanwhile another machine was supplied bearing Sl No. 08JGU062 also started giving problem. On our complaint to you, you came to our hospital and rectified the problem and gave a report stating that machine is in good working condition.

We have been requesting you to provide standby machine, but you have not provided the machine yet. Also you have been promising that you will return the machine after repair and we have been waiting for last Six months. Since you have not returned the machine, we are facing serious problem and it has effected the complete functioning of the OT and hospital.

Inspite of making it known to you about the problem, you do not seem to show any inclination to repair the above machine and when contacted you have been giving very evasive replies and oflate you have started ignoring us when we tried to reach you.

Kindly treat the matter as the most urgent and return the machine at the earliest after the repair on top priority.
