I cancel my contract but cause I moved they say ok but never did chicago

In octber 2004 I called to cancel my contract because I moved they told me to send a fax stating reason so I did send fax 10-29-04

Also I paid off the rest of contract about $600 dollars
but they keep sending bills just deducting from the amount I paid I called many times and they say that they sorry that they will cancel my account but they never did every time I call I asked why is my account open when I paid off the rest of the amount in order to cancel my contract every time they said they are sorry that they will stop my account but they do nothing
I want to know what to do to make them closed my account
they are monitoring address that I live no more also phone number they have don't exist anymore
please anyone can help
