Being charged for Damaged Device not Received after 7 months

On or about December I received an unusually high T-Mobile bill. Upon close review, I noticed that the bill included an additional one time charge in the amount of $223.37. The billing line item related to the charge describes the charge as "Asurion: Damaged Dev Not Recevd".

I immediately contacted T-Mobile to request an explanation of why I am being charged for the damaged headset that was returned to T-Mobile almost 7 months ago. I was informed that the damaged headset was never received by T-Mobile.

On or about May 24, I did received the replacement handset accompanied by:

1.instructions on how to return my damaged handset,
2. Shipping Box, and
3. Prepaid UPS shipping label (T-Mobile provided).

Following the instructions, on or about May 25, the content and package was prepared for shipment, the prepaid UPS shipping label was applied, and the package was dropped-off at the UPS Store. The copy of the Drop-Off Package Receipt was provided to T-Mobile. What else can I provide as a evidence of compliance.

All of my efforts to resolve the above matter amicably have been unsuccessful. T-Mobile was about to cancel my service few days ago. I have decided to pay this amount and take other available options, including taking them to small claim court and filing this complaint. Unfortunately, they refuse to provide the address of their serving agent in NYC so I may have problem starting small claim case against them.
