Credit Dropped to High Risk Over $40 Never Owned

I applied for a credit card and was denied, to my amazement, because I have always had perfect credit. I went online to get a credit report and found out I owed 40.00 for a credit I never had or owned. My credit dropped to high risk because they have not received payment in 120 days!
