Overcharges for bundle deal for almost 1 year! Carrollton texas

Last August I signed up for the 3-in-one bundle deal (internet, cable and phone land line (ICP) which was going to be $109.99/month for one year. I also did the required electronic signature on or the very next day. The first month I was overcharged I called and was told that those ICPs are actually billed separately and that everything will be OK. Don't worry.in the subsequent months I found out that my electronic signature did not show up and I do not show up as a "bundle" customer but I have regular rates.
To make a long story short, I have been overcharged as a "regular" rates customer every month since I signed up in August; I have called numerouse times and spoke with countless customer sevice reps and supervisors, I have electronically signed at least three times, and I have had my account credited with at least $450 in the past year or so.
The sad thing is that every time I call (latest about 3 weeks ago), I am told that I do not have the "bundle" promotion, that a ticket was sent to correct this issue, that IT WILL NOT HAPPEN AGAIN!
The cherry on my cake is that whan I electonically signed the second or the third time I actually signed for a 2-year contract at $99.99/month (and was credited for theese rates all the way back at some point). They said I should have read the contract before agreeing and signing on line. My bag. I am absolutely loathing the idea that I will have to fight with them for another year.
It has got to be a way to resolve this. My bill should be $99.99 per month plus a few boxes and small additions which puts it at approx. $126.99 taxes and fees, it should not be more than $160 a month. They are billing me almost $200 every month which I address every month.
Can anyone tell me how to resolve this?
Your input is much appreciated,
