Modification Approved Months Ago but Paperwork Never Generated

I have my home loan through Chase. I have been applying for a loan modification for 2 years. I have turned my paperwork 27 times. Every time I call I get a different answer. Two weeks ago I called and was told my modification was approved in June and the underwriter forgot to generate the paperwork. The representative told me they would expedite it. I should receive it by the end of the week. It still has never came!!! I need help, I want to keep my house!!! Please help me solve this.
