$250 Visa Gift Card Never Received After Two Year Agreement

I signed a 2YR Agreement in which after 90 days I would receive a 250.00 visa gift card and now I am getting the run around. Every time I call my call is either dropped or they can't find my offer. I see from my online research I am far from alone on this. I had my installation on 10/1.
