Charged $9.99 for MobTV not used, moves between phones monthly

I have been charged $9.99 for MobTV, when I have not been using it. I have told them the last several months that we are not using and to remove the charges. I have 5 phones on my account, one month it shows up on one phone, the next month it shows up on a different phone on my account.
